Confidentiality

Guided by the Health Insurance Portability and Accountability Act (HIPPA), contents of a student's Disability Services chart are confidential. The contents of the chart may include, but are not limited to, psychoeducational testing, neuropsychological evaluation, medical documentation, psychiatric reports, IEPs, and grades.

  • Parents/Guardians will not have authority to inspect, review and/or discuss the confidential information related to their child's case without written permission of the student except in instances where it is necessary to release such information in order to protect the health and wellbeing of the student or others.
  • In accordance with federal and state laws information contained in a student's DS chart will not be disclosed except in cases which require information to be released on a "need to know basis" or to protect the health and wellbeing of the student or others.
  • If a student desires for his or her confidential information to be released to an outside agency and/or professional, the student will need to provide written permission by completing a Confidential Release of Information form. Copies of this form are available in the DS Office.
  • The DS Office is not at liberty to disclose any information that has originated from another agency or office (i.e., psychological reports completed by a third party psychologist, medical records received from a third part physician, etc.).
